What Is MARPE?
MARPE stands for Miniscrew Assisted Rapid Palatal Expansion. It is a specialized orthodontic treatment that widens a narrow upper jaw, or palate, using a small expander anchored to the roof of the mouth with a handful of tiny, temporary mini-implants, also called TADs, or temporary anchorage devices. Those mini-implants engage directly with bone, allowing the appliance to gently and gradually separate the mid-palatal suture and widen the jaw without the need for surgery.
Traditional palatal expanders work well in younger children, whose palates are still easy to widen because the suture connecting the two halves of the upper jaw has not fully fused. Once that suture matures, typically in the mid to late teen years and in adulthood, a conventional expander often is not enough to move the bone on its own, and expansion has historically required a surgical procedure, known as SARPE, or surgically assisted rapid palatal expansion, to loosen the suture first. MARPE offers a way to achieve meaningful skeletal expansion in many of these older patients without surgery, using the added anchorage of the mini-implants to do the work a traditional expander cannot.
Who Is a Candidate for MARPE?
- Crowded teeth with insufficient room for a healthy bite
- A narrow, high-arched palate
- A posterior crossbite, where the upper back teeth sit inside the lower back teeth
- Airway restriction or breathing difficulties linked to a narrow upper jaw
- Cases where traditional expansion was attempted too late to be fully effective
Why Consider MARPE Instead of Surgery?
- No incisions and no surgical recovery, since the appliance is placed chairside
- Fewer risks than a surgical procedure performed under general anesthesia
- Lower cost and less disruption to daily life compared to a hospital-based surgery
- Effective skeletal, or bone-level, expansion rather than simply tipping the teeth, which can relapse more easily

What to Expect During MARPE Treatment
- Evaluation and planning. We assess palate width, suture maturity, and airway using a 3D scan and clinical exam to confirm MARPE is appropriate.
- Placement. The custom expander is fitted and the mini-implants are placed chairside, using local anesthesia. Most patients describe the process as similar to getting a filling.
- Activation. You will turn the expander a small amount each day, or as directed, gradually widening the palate over several weeks.
- Stabilization. Once the desired width is reached, the appliance stays in place for a period of time to allow new bone to fill in and stabilize the expansion.
- Follow-up orthodontics. MARPE is often one phase of a broader orthodontic plan to align the bite once the palate has been widened.

Why Choose Our Practice for MARPE
If a narrow palate is affecting your teen's bite, breathing, or facial development, MARPE may allow us to correct it without surgery. We will walk you through the evaluation, explain whether MARPE is a good fit, and outline exactly what treatment would involve, including cost and timeline, before moving forward.
